About Copilots In Care:

In the aging population, advance care plans (ACPs) are an important resource for patients to express their healthcare preferences before a crisis arises.

Today, 53% of seniors do not have an advance care plan. Doctors, who traditionally complete ACPs for their patients, are under intense time pressures and unable to facilitate complex conversations about end-of-life decisions. Without a plan, the patient's family and physicians default to over-treatment, leading to poor-quality care and unwanted costs.

To solve this problem, Copilots In Care re-envisioned the ACP completion process. With digital tools and a team of trained social workers, we help patients with a life-limiting illness (CHF, COPD, Cancer, and ESRD) reflect on and communicate their medical wishes to their family members and physicians.

Our goal is to help patients make the most of the time they have by empowering them as pilots of their care. Copilots in Care matches every patient with a social worker, the Copilot, who helps the patient understand their medical options and reflect on their values with their families. The Copilot then prepares the right documentation to ensure the patient’s medical wishes are executed.

We do not provide medical recommendations, nor do we try to direct care.  We simply facilitate communication to realign care so it matches patient goals. As a result, less healthcare costs are spent on unwanted treatment. We are partnering with health plans and ACOs to pilot our service. Simultaneously, we are refining our product to optimize the patient, family, physician, and copilot user experience.

We pride ourselves on being a small and scrappy team. Everyone is expected to take initiative and anticipate needs of the company, set goals, and accomplish tasks. A commitment to empathy and collaboration is fundamental to working in end-of-life care. We enter people's lives at a sacred and vulnerable time, and ask for their trust.  The patient is at the center of our work.  As a team, we err on the side of over-communicating and operate from a position of respect and trust for our colleagues. We put relationships before accomplishing tasks, and the best interests of the company ahead of anyone’s ego.  We are committed to open collaboration, constant learning, and failing fast.
